From 25th May, you will require identification when collecting a prescription or any paperwork from the practice.
You can bring a passport, driving licence or bank card.
If you ask someone to collect your prescription or paperwork on your behalf, we will need prior notice of this and a note will be added to your medical record.
We recently emailed all patients on our list asking for their consent for Jenner House Surgery to send emails for the purposes of communicating practice information, health campaigns, eligibility for vaccines and the patient newsletter, to date we have only had approximatley 450 replies.
If you have not provided your consent you will no longer receive the Practice newsletter or any further updates on practice business, including health campaigns.
If you would like to continue to receive emails, please email: [email protected], with your name, date of birth and a declaration of your consent to receive practice information, health campaigns and eligibility for vaccines and the patient newsletter from Jenner House Surgery. You can also pop into the Practice and let the Patient Services Team know.
